We are perhaps going somewhat OT here, but we use CBL (XBL actually, that
contains CBL) in Italy for business traffic with no significant FP issues.
If you refer to Fastweb, who indeed uses NAT for broadband pools so
that thousand of users are behind a single IP: blocking the
Fastweb residential NAT is completely safe and I think it is a "must".
There are no real mail servers coming out of those IPs.
They are permanently in CBL becayse they emit worms constantly
(there is always at least an infected users in thousands).
If there is a CBL FP in italian space, I will be interested to know
the IP to analyze the case (please mail me privately).
