Yeah, most people do - well, that's my guess anyway.

ISPs are in a different situation altogether, yes.

In my experience, rsync is good enough for down to 5 minute intervals
(depending on the amount of data). If you need less, yes, you'll have
to look at some sort of near-real-time update mechanism.
