Just use a generic howto about rbldnsd and refer to the "dnset" data  
type.

You can abtain this in two ways, IMHO:

1) using different A records to mean different scores
2) storing the score for the domain in the TXT record

The first will be easier to implement on the resolver/user side  
(thinking SpamAssassin), but requires to use just a bunch of  
different predefined scores.
The second will be much more flexible (you can set the score you want  
for every single record) but may be harder to implement...
